Axial Exchange Wins DHHS's $25K “Health 2.0” Challenge with its Care Transition Suite

It’s been announced by the Department of Health and Human Services (DHHS) that Axial Exchange has won it’s $25K “Health 2.0″ challenge with its innovative Care Transition Suite designed to ensure a “safe transition from hospital to home.”

Axial’s “Care Transition Suite” works by coordinating data management among all participants — including emergency responders, physicians, hospitals, etc. – during the transition of a patient back and forth between their home and various clinical settings. The solution involves three of the company’s products; Axial EMS, Axial Alerts, and Axial Patient, to provide an unprecedented view of the patient’s entire care experience.
