EHR Usability Gaps -- ONC and AHRQ Identify Serious Shortcomings of Certified EHRs

The Health Information Technology for Economic and Clinical Health (HITECH) Act provided providers with a significant financial incentive to increase the adoption and use of EHRs. EHR vendors were required to conduct and report on a summative usability evaluation of their system as part of the Stage 2 Meaningful Use program (The ONC 2014 Edition Certification). However, a recent report funded by the Agency for Healthcare Research and Quality (AHRQ), identified several “issues” with the certified EHR vendors in the processes, practices and use of standards and best practices with regard to usability and human factors.

When big health insurers propose mergers, it makes for good antitrust enforcement theater to try to block them. However, if government officials want to address anti-competitive activities that have a dramatically bigger impact, they should shift their focus to local market provider M&A activity that consistently show prices increase after the deal is done. However, the most rapacious, anti-competitive practices I’ve seen in my entire career have come from hospitals–frequently from tax-exempt “nonprofits” that would make John D. Rockefeller blush with their brutal actions. The combined impact has created a middle class economic depression that has driven populist presidential campaign success, which was highlighted in a recently released Brookings study.

Ripple: Making Waves in Healthcare IT

The Ripple Program, based out of Leeds and building upon the lessons learned from the Leeds Care Record, has recently been set up to positively disrupt health and social care towards those ends. Importantly the ethos of “open” is at the heart of the work and for very good reason. It is clear that interoperability between Health IT systems will drive real change but, what is even clearer is that only an open source approach will positively disrupt this health and social care landscape across the NHS and across the globe. Funded by NHS England and hosted by Leeds City Council, the focus of Ripple is to support health and social care organizations by providing six open source elements which can be used individually, in combination or as a whole, and are consistent patterns of need when embarking on an work towards healthcare improvement with information technology.
