The VA Offers New Model of Healthcare for Veterans: Patient-Aligned Care Teams
The Department of Veterans Affairs is now offering a new streamlined healthcare delivery system to our veterans called Patient Aligned Care Teams.
So what does this mean? This means that the VA is working on bringing primary healthcare to veterans to the next level. What they are doing is aligning all of the healthcare pieces together; to make the healthcare delivery system more manageable, cohesive, and highly functional by using teams called Patient Aligned Care Team or PACT.
So what is the role of the Patient Aligned Care Team? The VA by creating a Patient Aligned Care Team is embracing what is known as the patient centered medical home model, which is a model of care offered by a team healthcare professsionals. A personal provider leads the team and works on providing continous and coordinated care throughout a veteran’s lifetime.
